Start Using the Open Door Patient Portal in 3 Easy Steps

Provide us with an email address where we can contact you.
Activate your Patient Portal account by clicking the link in the activation email you receive. Then, enter the provided username and temporary password.
Change the temporary password to make it your own. This will now be the safe, protected username and password you use anytime you wish to access your health information.
